句子是：
It was at the very beginning that Mr.Smith made a decision that we should send for a doctor.
这个句子里面原本有两个空,其中一个是decision后面的That的位置,有两个选择,一个是Which,一个是That,但是只能用That不能用Which,我们老师也解释不出个所以然,大家帮忙啊!

第一个that和it was构成强调句型.第二个that是同位语从句的引导词,同位语从句只能用that来引导.
